ATTENTION: If you are registering and would prefer not to pay online or do not have a credit/debit card, DO NOT fill out this form. You may pay the $25.00 tryout fee in advance over Venmo (listed below) and you MUST leave the player's first and last name in the comment. If you choose this method, check in for tryouts will be slower and you will be required to fill out a separate form at the door with registration information. 

PARENTS: Please come in to check in your player rather than dropping them off because you will be required to sign a waiver! If you have a conflict contact us and we will find a solution

2024-2025 NEVA TRYOUT REGISTRATION FORM

IMPORTANT MESSAGE TO PARENTS AND PLAYERS:​

 

Tryout entry fee is $25 +cc fees (one-time) and includes a NEVA Club T-Shirt. Filling out the registration form will direct you to a secure payment platform. If you registered prior to this date and have not yet paid, you may pay in advance over Venmo, @northerneliteva. Please leave the player's name in the comment. Payments will also be accepted on the day of tryouts, either by check  (made out to Lorenzo Gasper), $25 cash, or over Venmo.
